If you do not have access to the Patient Portal, follow these instructions to Register.

1. Provide your email address to Dr. Paglia's office. If you do not
   have an email address, Dr. Paglia's office can assist you with obtaining an
   email address.
2. You will receive an email from Practice Fusion with instructions
   for accessing your Health Records online
3. You can register by using one of the following methods:
   
   • Using the temporary PIN number in the email sent to you from Practice Fusion
   • Or using your phone number recorded by Dr. Paglia's office
